Hatchment and Atchievement

Can any one of the readers of "NOTES AND QUERIES" tell me how comes the corruption *hatchment* from *atchievement*? Ought the English word to be spelt with a *t*, or thus, *achievement*? Why are hatchments put up in churches and on houses?
